WHAT TO BRING TO APPOINTMENT

• Prior medical records and/or x-rays. (If seen in Emergency Room and they did not give you a copy, you can obtain one from the radiology department)
• Insurance information, your ID card helps determine your Copay. Please bring correct change if possible. We do take Visa/MasterCard.
• If shoulder or knee evaluation, your exam is much easier if you are able to wear or change into a tank top or shoulder or shorts for knee exam.
